Closure is gated in code, so a closed record can never hide open risk.

A degraded barrier needs a linked corrective action, a corrective action is not done until it is proven effective, and a risk waiver is two-person co-signed so it survives an audit. Free-text systems let a user hand-type a green status no one checked.
